NPHI Welcomes Midland Care Connection

[WASHINGTON D.C.] April 24, 2024 – The National Partnership for Healthcare and Hospice Innovation (NPHI), the national voice for not-for-profit hospice and palliative care providers, proudly announces the addition of Midland Care Connection to its esteemed community of almost 110 not-for-profit, mission-driven, advanced illness, hospice, and palliative care providers.

Midland Care Connection, a pioneering and compassionate not-for-profit organization, has been at the forefront of addressing the health and aging challenges in the Kansas community since its establishment in 1978. Committed to ensuring that the seriously ill can live with dignity and respect, they offer an integrated system of care that addresses the social, physical, and spiritual needs of their clients. Midland Care strives to keep older adults at home and independent longer, embodying their mission to meet individual needs through innovative care solutions.

Midland Care Connection offers a comprehensive range of services designed to support individuals and families throughout their health and aging journey. These services include hospice, palliative care, grief and loss counseling services, home health, meals on wheels, and their program of all-inclusive care for the elderly (PACE). Their holistic approach to care ensures that each person receives personalized support tailored to their unique needs and preferences. In addition to these core services, Midland Care Connection is partner and Manager of Community Care Hospice, a new joint venture in Central Kansas with not-for-profit senior living communities, and is also a co-founder and member of Responsive Care Solutions, a unique accountable care organization participating in the Medicare Shared Savings Program focused on serving frail elderly populations.
